   Hi There, A few things come to mind. It is unlikely he has heart disease, unless he is using cocaine or amphetamines. It sounds more like stomach spasms from reflux and gastritis, could also be pancreatitis or gall bladder problems. There isn't one single blood test to hang your hat on for gall bladder. There are blood tests for pancreatitis, none for reflux, gastritis. Overall, I would have him screened for gall bladder with an ultrasound. Then, give him a trial of one of the acid blockers, like over the counter prilosec 1 per day or Zantac 75, 2 pills twice a day. If symptoms persist past that, then repeat the blood tests for pancreatitis (amylase and lipase) and possibly a referral to a GI doctor. You didn't mention alcohol history, but that may be an aggravating factor and you already know that cigarettes are. Hope it helps
