The Nikkah in emergency / special conditions

If a girl and a boy (both over 18) accepted each other as husband and wife with no Witnesses for the reason of avoiding a Sin and later told people that they got married. Did Nikah happen or not?
Secondly two witnesses are required in Nikah, Can the Qazi who asks for being husband and wife be one of the witnesses or we require two witnesses other than Qazi?

    As per my knowledge the presence of witnesses is extremely necessary because Islam put heavy emphasis on declaring the marriage in front of people. Logically if you see that if it is allowed to accept each other as husband and wife without witnesses, it could be a source of all evils. Coz any boy and girl who may establish exual relationship can later declare that they had been married and due to unavoidable circumstances they could not manage a nikkah nama and witnesses.
