   You will have to follow four simple steps to activate your optimum reward card.

    

   First you will have to visit the Optimum Rewards card activation page by accessing their official webpage. There you will be asked to type the account number printed across the bottom of the front of your Optimum Rewards card in the box provided. You will have to enter your name the way it appears on your card in the box provided.
   Finally type your billing address ZIP code in the box provided. Click "Submit" to complete your Optimum Rewards card activation.
