    To set up a Direct Debit send the Direct Debit form to HMRC. The address is on the form. Starting self-employment should download ‘Direct Debit form, CA5601’ here: http://www.hmrc.gov.uk/pdfs/ca5601.pdf Already self-employed should download ‘Direct Debit form, CA5611’ here: http://www.hmrc.gov.uk/nic/ca5611.pdf Allow 21 days to set it up. HMRC will tell you in writing before taking the first payment. Payments appear on your statement as ‘HMRC NI-DD’.
