2006 Chevy 2500HD 6.0 #4 & 5 Cyl Misfire

I have a 2006 chevy 2500hd 6.0 I cant figure out why it is misfiring? I replaced all coils, wires, plugs, injectors and it still misfires only at idle and has a small rattle

    had the same problem with 2004 van changed wires coils come to find out had proble with fuel rail on passenger side fix problem the old one had something loose in it hope this helps you
